LogistiCare Transportation to Medical Appointments

Wayne, Oakland and Macomb residents can contact LogistiCare to request a ride to medical appointments at 866-569-1902.  All rides  must be for a medical appointment or dialysis treatment.  YOU MUST CALL AT LEAST 2 BUSINESS DAYS IN ADVANCE   You need to have your Medicaid ID and the name, address and phone number of your medical provider.

If in a Medicaid HMO then contact them for transportation.  Call LogistiCare for rides to the dentist, substance abuse and mental health services if you have no other way to get a ride.
